How It Works
NHS pay in England is structured under the Agenda for Change (AfC) framework, which groups roles into nine pay bands. Each band has multiple pay points (incremental steps) that staff progress through annually based on length of service. For 2026/27, Band 5 (newly qualified nurses) starts at around £29,970 rising to £36,483.
In addition to basic pay, many NHS staff receive High Cost Area Supplements (HCAS) for working in London and surrounding areas: Inner London (20% up to £8,096), Outer London (15% up to £5,329) and Fringe (5% up to £1,929). Unsociable hours, on-call and overtime attract additional premiums.
This calculator takes your band, pay point and location to show your total gross pay including any HCAS. It then calculates take-home pay after income tax, NI and NHS Pension contributions (the NHS Pension scheme has tiered contribution rates from 5.2% to 13.5% based on pensionable pay).
How NHS pay (Agenda for Change) works. Most NHS staff (nurses, allied health professionals, admin, support staff — but NOT doctors) are paid via Agenda for Change pay scales. Bands 1-9 reflect job role and responsibility. Each band has multiple pay points — annual increment automatic until top of band. 2026/27 example: Band 5 newly qualified nurse starts £29,970, top of band £36,483 after 4 years. Pay rises announced annually (typically April implementation).
Doctor pay structure. Junior doctors: foundation year 1 (FY1) £39,500, FY2 £45,800, specialty registrar starting £53,000, top registrar £80,500. Consultant: £105,500-£141,000 base, plus clinical excellence awards. GPs: salaried £77,000-£117,000 base, partner profit share variable £100-£250k+. Plus NHS pension (one of UK's best — 1/54th career average with 1.5% revaluation). Most NHS doctors earn 30-40% of total compensation in pension accrual.
London weighting and unsocial hours. Inner London weighting: 20% supplement (Bands 5+ get £3,000-£8,000+ extra). Outer London: 15%. Fringe (within 30 miles of London): 5%. Unsocial hours premium (nights/weekends): Band 1-3 = 40-50% supplement; Band 4-7 = 30-40%; Band 8+ = 25-30%. On-call rates from £30-£70 per session plus call-out fees. Many NHS roles double pay from base via shift work patterns.
Pay progression and promotion. Annual increments automatic within band (top of band reached in 4-7 years typically). Promotion to higher band requires job change — apply for higher-band roles internally. Cross-band moves: experience counts toward starting point in new band. Specialist progression: nurses Band 5→Band 6 (within 5 years for ambitious); Band 6 to Band 7 typically requires masters degree and specialist role. NHS staff turnover average 12% — significant retention concerns.
